Community Support Workers – Children and Young People

Salary: £21,898 - £23,463
Location: Northwest England
Summary of Function: To provide ongoing support and delegated health care tasks for children and young people with complex health and social care needs living at home. Maintain high standards of support and team working. And most importantly to enable our service users to have fun and get the most our of their daily lives!!

 

Main Duties Responsibilities

(N.B. The following is indicative of the range of tasks undertaken by the post holder but is not exhaustive. The role is likely to evolve as the business develops and is therefore subject to regular review).

Clinical:

  1. To be alert to and notify changes in the service user’ needs to the Line Manager to ensure appropriate standards of care/support are maintained at all times
  2. Ensure that the clinical needs of the service user are met through the effective use of Individual Support Plans and associated risk assessments/management plans
  3. Ensure that all interventions are within a framework of person centered care
  4. Actively participate in the direct care/support of the service user
  5. Actively participate in meeting the social, emotional and psychological needs of the service user
  6. Assist in identifying where the care service can be developed to enhance the overall service and add value to the Individual Support Plan
  7. Assist the Service User in presenting their views in case reviews and any meetings as appropriate
  8. Complete accurate and timely digital care records of all tasks undertaken

Professional:

  1. Maintain professional boundaries for all matters of a care delivery
  2. Work at all times within the policies and procedures of the company
  3. Lead by example through attention to professional conduct
  4. Always work in line with the Individual Support Plans

Training and Development:

  1. Attend all training days as laid down by the company and identified within your individualised Training Plan
  2. Complete all E-Learning within the agreed time frames
  3. Be accountable for your own competence and inform line manager of any updates/refreshers required
  4. Assist in the mentoring/shadowing of new CSWs

General:

  1. Develop and maintain open and honest communication with the service user and their family
  2. Always act in a manner to promote the health, safety and welfare of self and others
  3. Always act in a manner to positively promote the company and its ethos
  4. Adhere to the company’s policies and procedures
  5. Maintain a clean and tidy home for the child where the Individual Support Plan requires
  6. Promote the independence, as much as possible, of the service user
  7. Ensure the service user receives a nutritional diet in conjunction with their health needs, choices dietary Care Plan
  8. Drive the service user’s vehicle where needed in line with company policy

Qualifications:

  • NVQ L2/Diploma Health and Social Care or NVQ L3/Diploma Children and Young People, willing to work towards – Desirable

Experience:

  • Experience of caring for people with complex needs – Desirable
  • Working with people with additional needs – Essential
